OAK Disability Support provides person-centred disability supports designed to help individuals build independence, confidence and connection in their everyday lives.
 

Our supports include community access and participation, daily living and life skills development, personal care, meal preparation, capacity building and assistance with everyday activities.
 

We also provide domestic assistance, household cleaning and practical in-home support.

Our disability support services are designed for people living with disability who require assistance with everyday activities, independence and community participation.
 

We welcome enquiries directly from participants, family members, nominees, Support Coordinators and other members of a participant's support network.

Yes. We provide supports to NDIS participants where the requested service aligns with our scope of supports and service capabilities.
 

Our team can discuss your funding arrangements and the type of support you are looking for during the enquiry and intake process.

Our NDIS support pricing is charged in accordance with the applicable NDIS Pricing Arrangements and Price Limits. Pricing varies depending on the type of support required, when the support is delivered and the individual service requirements.
 

All applicable pricing and agreed supports are clearly outlined in the Scope of Supports issued with your Service Agreement.

OAK Disability Support provides services across a range of Brisbane and surrounding regions, including the Redlands and Bayside, Brisbane, Logan, Ipswich, Moreton Bay and surrounding suburbs.

Quality and safety are at the heart of the supports we provide. OAK Disability Support has policies, procedures and systems in place to support safe, consistent and high-quality service delivery.
 

Our workers complete relevant screening, onboarding and training requirements and are supported to understand their responsibilities when providing services. We also regularly review our practices, manage risks, respond to incidents and feedback, and look for opportunities to continuously improve.
 

Most importantly, we listen to the people we support. By maintaining open communication and taking a person-centred approach, we aim to provide supports that are safe, respectful, reliable and responsive to each individual's needs.

Getting started is simple. Reach out to our team by phone, email or through our online enquiry form and tell us a little about the support you are looking for.
 

From there, we will discuss your needs, arrange a Support Planning Consultation where appropriate, prepare your Service Agreement and Scope of Supports, and work with you to organise the commencement of services.

A Support Planning Consultation may be held over the phone, video call, or via email. It gives us the opportunity to get to know you, understand your goals, preferences and support needs, and answer any questions you may have about OAK.
 

It is also an opportunity for you to learn more about our approach and discuss what you are looking for from your supports before services commence.

Yes. Before ongoing services commence, you will receive a Service Agreement and Scope of Supports outlining the agreed services, responsibilities, pricing and other important service information.

We encourage you to review these documents and ask us any questions before signing.

Absolutely. With the appropriate consent and involvement from the participant, we are happy to communicate with Support Coordinators, family members, nominees and other members of your support network.
 

We believe strong communication and collaboration can play an important role in providing consistent, person-centred support.

Yes. Your supports should reflect your individual needs, preferences and goals.
 

During the intake process, we will work with you to understand what support you require and develop an agreed Scope of Supports.
Services must remain within OAK Disability Support's scope of practice and the agreed services outlined in your Service Agreement.

Yes. We can support individuals to access their community, attend appointments and activities, participate in social opportunities and build greater confidence when engaging in everyday community life.
 

Your support will be tailored around your individual goals, preferences and support needs.

Yes. Our daily living and life skills supports are focused on building confidence, capacity and greater independence.
 

This may include support with meal preparation, household routines, planning and organisation, everyday tasks and developing practical skills that are meaningful to your individual goals.

Yes. We can provide personal care and assistance with daily personal activities within our scope of supports.
 

We will discuss your individual support needs during the intake process to ensure we understand the assistance required and can safely and appropriately meet your needs.

Yes. We also provide domestic assistance and household cleaning services, including support for NDIS participants.
 

Our cleaning services are delivered with care, consistency and attention to detail, with services tailored to the agreed household support requirements.

We understand that plans can change. Cancellation requirements and any applicable cancellation charges are outlined in your Service Agreement and Scope of Supports.

Yes. We understand that needs, goals and circumstances can change over time.
 

If you would like to discuss changes to your supports, contact our team. We can review your current arrangements and discuss whether changes can be made to your agreed Scope of Supports.

Yes. The safety and wellbeing of the people we support is a priority at OAK Disability Support.
 

Our support workers are required to complete relevant pre-employment screening and compliance checks, including an NDIS Worker Screening Check. We also verify relevant qualifications, experience and employment requirements based on the supports each worker will be providing.
 

In line with NDIS requirements and the NDIS Code of Conduct, our workers complete onboarding and ongoing training to ensure they understand their responsibilities and are equipped to provide safe, respectful and person-centred supports.
 

We are committed to maintaining a capable, compliant and supported workforce so you can feel confident in the people providing your supports.

Where possible, we aim to provide consistency and build strong, trusted support relationships. This means we try to place you with the same support worker each service.
 

Support worker availability, leave and scheduling requirements may occasionally mean an alternative worker is required. Where this occurs, our team will communicate with you and work to maintain continuity of support wherever possible.

We consider a range of factors when matching support workers, including support needs, personality, preferences, location, availability and the skills required to deliver the agreed supports.
 

Our goal is to create positive and respectful support relationships where participants feel comfortable, heard and supported.

We want you to feel safe and comfortable with the people providing your support.

We will listen to your concerns and work with you to find an appropriate way forward.

We welcome feedback and take all concerns and complaints seriously. Feedback helps us understand what we are doing well, identify opportunities for improvement and continue to provide safe, quality supports.

You may also choose to have a family member, advocate, nominee or support person assist you throughout the process.
 

We aim to ensure complaints are handled respectfully, fairly and in a way that keeps you informed, safe and involved. Raising a concern or making a complaint will not negatively affect the supports you receive from OAK Disability Support.
 

You also have the right to raise a concern or make a complaint directly with the NDIS Quality and Safeguards Commission about the quality or safety of NDIS supports or services. An independent disability advocate can also assist you to make sure your voice is heard.
 

We encourage open communication and will work to respond to concerns promptly, respectfully and in line with our complaints management process and NDIS requirements.

We take privacy, dignity and confidentiality seriously. Personal information is collected, used and stored in accordance with applicable privacy requirements and our information management processes.
 

We only share information where appropriate, authorised or legally required.
